1: A health certificate or health record with appropriate vaccination stamps, dates, or a
Vet signature. Most puppies should be vaccinated 1-2 times by the time they are 12
weeks old
2: A record of tattoo or microchip and the correct number. If it is a tattoo, be sure to ask
where the tattoo is located, and look to make sure the number matches up.
3: A health certificate or health record with appropriate deworming information. Most
puppies should be dewormed 1-2 times by the time they are 12 weeks old.
4: Be sure to record your puppies parents Registration names, numbers, & OFA/OVC
numbers (if applicable).
5: Gather your puppies birth date, & litter size and remember to ask the breeder to
supply you with a 3-5 generation pedigree.
6: Ask the breeder about getting a sample baggy of the puppy food that they were
feeding, and ask for recommendations. This bag of food will come in handy if you are
planning to switch brand-names, as you can mix it more gradually with your new food.
7: If you have purchased a pet, make sure you receive a copy of the signed
non-breeding contract.
8: A SIGNED (by the breeder & yourself) copy of the written contract/health &
temperament guarantee.
Bringing Puppy Home - What you will need:

park.

regret it. )

great!

12: An ID tag for your pup's collar with microchip or tattoo information, your name &
